GENERAL RELEASE AND INDEMNITY

I. Introduction

Samarth Life Management Private limited (the “Provider” or “Samarth”) offers holistic, technologyenabled caregiving solution that supports seniors living at home. MSCI Services Private Limited (together
with its affiliates, the “Company”) has partnered with this Provider to make available such various voluntary
elder care services (the “Benefit”) to its employees or contractors and their elderly relatives (each such
person, a “Beneficiary” and collectively, the “Beneficiaries”). Employee will pay the full cost of the fees of
the Benefit that a Beneficiary elects and/or receives in accordance with and subject to the Company’s
policies, as amended or updated by the Company from time to time. It is acknowledged and agreed that
participation in the Benefit is at the sole election of each Beneficiary and that the Company has played no
part in the Beneficiary’s decision to avail and use the Benefit. All such decisions are taken voluntarily by
(or on behalf of) the Beneficiaries at their own election, cost and at their own risk.

II. Assumption of Risks; Release of Liability; and Waiver of Claims

In consideration of the Company making available this Benefit and giving me and/or my elderly relatives the
opportunity to avail myself and them of it, if I so choose, I agree on behalf of myself and/or my elderly
relatives as follows:

(a) I fully understand and acknowledge that:

(i) I have chosen to enrol in the Benefit of my own volition.

(ii) I have reviewed the Benefit myself and have taken an informed decision after confirming the relevant
parameters, to avail of the Benefit.

(iii) I am fully aware of or have had the opportunity to become aware of the risks and dangers that do or may
exist in connection with using the Benefit.

(iv) I hereby assume and take on all risks and dangers and all responsibility for my decision to participate in
(and have my children participate in) the Benefit.

(v) I hereby agree and acknowledge that I will bear the entire cost of this Benefit at my own will.

(b) In furtherance of the foregoing, to the maximum extent permitted by applicable law, I hereby voluntarily
agree, on behalf of myself, my relatives, my children and personal representatives, my estate and my heirs,
to release, waive, discharge, and hold harmless the Company, its parent, affiliates, subsidiaries, agents,
shareholders, officers and employees from any and all manner of claims, demands, causes of action,
obligations, losses, damages or liabilities of every kind and nature whatsoever, at law or in equity, which do
or may arise in connection with, related to, or as a result of my use of the Benefit including, but not limited
to, illness, injury, physical or emotional harm, disability, dismemberment, death, property damage or loss
resulting therefrom, and any claim for attorney’s fees or costs.

(c) This is a complete and irrevocable release and waiver of liability of the Company and the aforesaid
parties.

I CONFIRM THAT I HAVE READ, FULLY UNDERSTAND, AND AGREE TO THE PROVISIONS OF MSCI
GENERAL RELEASE AND INDEMNITY.
